A web based application developed for maintaining the information related to spine patients in a hospital. The application includes various forms to manage the patient information: 

• Preoperative: Maintains information related to the patient before operation.
• Intraoperative: Maintains information related to the patient during operation.
• Postoperative: Maintains information related to the patient after discharge from the hospital. It preserves information about the care taken to the patient for 24months after discharge.
• Questionnaires: Maintains information filled by the patient or his family member regarding hospital services.
